Guer Pharmaceuticals is a virtual development company built for rare diseases. We find and advance orphan drug candidates with two engines working in sequence — artificial intelligence to search at landscape scale, and biostatistics to judge what the search turns up.
AI finds the candidates. Statistics decides which ones deserve capital.
Guer Pharmaceuticals, Inc. is a Maryland-based company dedicated to developing medicines for rare, or "orphan," diseases — conditions that affect small patient populations and are often left behind by large development pipelines. Our development engine is AI-driven: we use machine intelligence to survey the orphan-drug landscape, score and prioritize shelved or undervalued candidates, and design focused programs sized to the disease rather than to a blockbuster template. But no candidate advances on the algorithm's word alone — each shortlisted asset must then clear a statistical gate, where the evidence behind it is graded and its probability of development success is quantified before we commit.
Our name is a nod to the Chinese word for "orphan" — a reminder, in every program we run, of the patients these medicines are for.
Every Guer program passes through the same four steps. The first two are powered by artificial intelligence; the third — the one most AI pipelines skip — is powered by statistics. Capital only moves at the end.
We continuously map the global orphan-drug landscape: shelved programs, academic discoveries, distressed pipelines, and non-core assets — including the ones conventional review stopped reading.
Machine intelligence scores every candidate on scientific plausibility, regulatory path, and commercial fit, compressing a landscape of thousands into a shortlist worth a second look.
The gate. We grade the strength of the evidence, audit the quality of prior trial data, and quantify each program's predicted probability of success — with endpoint and sample-size analysis that tests whether a viable trial can actually be run.
Only candidates that clear both engines earn capital. Guer acquires or licenses them in, then advances development as sponsor through its virtual model and specialized partners.
Why a statistical gate? AI is good at finding patterns; it is not accountable for them. In rare diseases — small trials, thin data, no second chances — a disciplined statistical read of the evidence is the difference between a calculated risk and an expensive hope.
The process above runs on a deliberately small operating model. Guer stays lean by design: the company itself is the development sponsor, the two-engine evaluation is run in-house, and every laboratory and specialized function is performed by external partners chosen for the program at hand.
What stays in-house is judgment. Biostatistics and data science — candidate selection, evidence grading, trial design, go / no-go calls — are the company's core discipline, not an outsourced afterthought.
Guer holds the assets and owns the development decisions — strategy, regulatory path, and program governance stay in-house.
Preclinical, laboratory, clinical, and manufacturing work is executed entirely by specialized contract partners across the US, Europe, and Asia, matched to each asset's science — without fixed overhead.
Our focus is rare and orphan diseases where a small, disciplined development program can change the outlook for patients who currently have few or no options — and where AI screening paired with statistical validation can surface value that conventional review misses. We evaluate in-licensing and acquisition opportunities in assets with a clear orphan designation path and a development plan that fits our virtual model, and we welcome approaches from academic groups, foundations, and companies seeking a committed development home for shelved or non-core rare-disease assets.
